I have read a copy of the facility use guidelines, and agree to follow them.



· Person making the request assumes responsibility for the group.

This includes maintaining order, leaving the library as it was, and respecting others and property.

· If the furniture is rearranged, it should be returned to the original arrangement at the end of the lesson, leaving it ready for the next class. See attached floor plan for placement of tables, chairs, etc.

· Classes will need to be responsible for the use and care of audio/visual. The equipment in the library is intended for the teachers and students of Mary Immaculate. Extension cords etc will be provided upon request.

· Students are not to use the library computers during after-school activities.

· If the facilities are used for Meetings after normal library hours, please turn out lights and secure (lock) the library.

· During Class visits, younger siblings are not allowed unattended in the library. If your Class includes adults, young children are not allowed unattended in the library.

· All paper is to be placed in the recycling bins provided.

· Handle books with care and respect and return to shelves correctly.

· Furniture and/or equipment from the Library may not be taken into other areas of the school.

· Permission to use the Library may be withheld from Students failing to comply with the Library Use Agreement and from any Class that damages the room, carpet, equipment, or furniture, or causes a disturbance.
